Case Management at Bienestar provides services to indigent persons with HIV/AIDS residing within Los Angeles County. Case Management provides assistance to eligible individuals living with HIV/AIDS and their family to facilitate access to services and support to coordinate these services. Case Managers assess the client’s needs to developed a comprehensive service plan to provide clients with information and referrals to a variety of services ranging from, access and adherence to primary healthcare care, housing, food bank, mental health, substance abuse programs and issues non HIV related. The goal of case management program is to increase client’s self-sufferance by providing quality services in the most efficient and effective manner possible to individuals with multiple complex needs.
